The importance of effective communication within Doctor- Patient relationships. Upon setting out on this placement, it was my intention to study the communication methods and the effectiveness of such by doctors within the multidisciplinary team in order to prepare this assignment. Whilst on placement I was assigned to an On Call Registrar assigned to many consultants at the time. This wasn’t particularly ideal as continued contact with patients was not available to me, however there was one patient I recall whom I gained a lot of insight from. We were called to the extension ward of AE where we were presented with a young woman, 19 years of age. She had been sent across from her GP with severe posterior flank pain. A urine test was†¦show more content†¦We later found out that a urine sample had in fact been obtained at the GP surgery earlier that day, which the patient had falsely claimed it had not. What stood out about this situation to me was that this patient had withheld this vital information from the registrar. It could not be said whether this had been intentional by the patient, however it was decided not to discuss it further with the patient, as this may have hindered the trust and rapport that had already been built. It may have been due to a breakdown in communication between the patient and her GP, but because of the nature of the role in which the registrar had acquired the final outcome of this case, it was not witnessed. Upon our withdrawal from this patient and AE department it was felt that the patient felt a lot happier and more at ease with the care that she had been given so far and what was planned for her. We felt confident in our discussions, that we made the right choice and that this was the right course of action for this particular patient. Initially my feelings regarding this incident were quite harsh and somewhat irrational. My only thoughts were why had this patient lied and concluded that she may be amplifying her symptoms or potentially attention seeking. However, this did not affect my treatment towards this patient and she received the investigation and treatment she needed in which to recover as instructed by the registrar. Buddhism has a very long existence and history, starting in about 565 B.C. with the birth of Siddhartha Gautama. The religion has guidelines in two forms in which Buddhist followers must follow. During the school day, businesses use children as willing subjects of market research. They do such things as taste tests and answer opinion polls. This is a difficult subject to pick a definite side on, because both sides have good arguments. This paper will explain both sides and come up with a solution. Almost anyone will agree that exploiting children is bad. Students are in school to learn and taking polls and taste tests are in no way an education. Through this market research, the companies are merely trying to find an easier way to target children. This situation is similar to the calls that people get at home. It is dinnertime and the phone rings. Mom jumps up from the dinner table and answers the phone. It is a market research firm and they would like to ask Mom a few questions. Mom is angered by the call and hangs up. This is a prime example of market research and the inconvenience it causes. Advertisers realize that schools are the perfect places to develop new markets. Kids can not hang up the phone or change the channel. Schools used to be the only place where children were not targeted. Children are entitled to have a place where they can feel no one will be trying to sell them something. If children are constantly exposed to this kind of market research they will begin to think that their education is not important. They will not be able to focus in class, because they will be waiting for the next poll or taste test. This is detrimental to do during class time. It is commendable that the school asked the parents for permission before letting the companies come in to do their research. Unfortunately, this market research has opened the doors to other marketing. Direct marketing to kids has already begun in some schools. Kids in elementary school and high school walk around with Nestle and Calvin Klein book covers. Soon students will be sitting in Pepsi owned chairs, at Pizza Hut desks, with Oreo school bags, looking at a Campbell's chalkboard. As funny as this may sound, it is a definite possibility. According to a recent Time magazine article, the district administrators in Plymouth, Mich. are considering auctioning school names to the highest bidding corporation. Imagine sending kids off to McDonald's Elementary or Coca-Cola High. It may not end there. The situation may snowball into something terribly worse. What if they begin to recruit our children as actual workers? Even if the work was fun or educational, that is unacceptable. Childho od should be the least stressful time in life and work will make the children hardened with the real world. These market research companies should be taken out of the schools and kept away from the children. Most of the emissions from fossil fuels come from power plants, but second ranked is transportation. Maybe you have already adjusted many aspects of your lifestyle to reduce your carbon footprint, including installing LED lights, turning down the thermostat, and eating less meat. For many of us, especially in rural areas, bicycling or walking to school and to work may not be an option, and public transportation may simply not be available. Do not fret; there are still actions you can take to lower the pollution and greenhouse gas emissions you produce when driving. Fuel Economy vs. Emissions We generally assume a vehicle with better fuel economy will also release fewer harmful emissions, including greenhouse gases. The correlation generally holds true, with a few caveats. Decades-old vehicles were built under much more relaxed emissions regulations and can be prodigious pollution producers despite a relatively modest thirst for fuel. Similarly, you may be getting 80 miles per gallon on that old two-stroke scooter, but that smoke will contain much more harmful pollutants, much of it from partially burned gasoline. And then there are the cars with emission control systems releasing illegal amounts of pollution, like those finger-pointed during the infamous Volkswagen small diesel engine scandal. The obvious place to start to reduce emissions, of course, is by choosing a modern vehicle with the best possible fuel economy. Models can be compared using a handy web tool put together by the U.S. Department of Energy (DOE). Be realistic about your needs: how many times a year will you really need a pick-up truck, sport-utility vehicle, or minivan? Performance is another fuel economy killer, but if you really want a sportier car, favor a four-cylinder model with a turbocharger instead of a larger six or eight (or twelve!) cylinder car. The turbo kicks in on demand, with the more frugal four cylinders doing the work the rest of the time. Manual vs. Automatic Not so long ago manual transmissions provided better fuel economy than automatic transmissions. It was a good excuse for those who love to row their own gears but modern automatic transmissions, which now have 5, 6, and even more gears, provide better mileage. Continuous Variable Transmissions (CVT) are even better at maintaining the engine’s revolutions at the right speed, beating even the most skilled stick-shift enthusiasts. Older Car, Newer Car Older cars were designed and constructed in the context of emission regulations that were much less restrictive than they are today. Often the emissions control system is a fault, which means you are polluting more than usual. Bring the car to your mechanic for a proper diagnostic, it may save you from more expensive damage later on. Car Modifications After-market performance modifications abound in some types of cars – louder exhaust pipes, modified air intakes, reprogrammed fuel injection. All those features increase your engine’s fuel needs, so get rid of them or better yet don’t install them in the first place. Larger tires and suspension lifts need to go too.
